Critical thinking and creativity in nursing: Learners' perspectives

Background Although the development of critical thinking and the development of creativity are major areas in nursing programme, little has been explored about learners' perspectives towards these two concepts, especially in Chinese contexts. Objectives This study aimed to reveal nursing learners' perspectives on creativity and critical thinking. Design Qualitative data collection methods were adopted, namely group interviews and concept map drawings. Settings The process of data collection was conducted in private rooms at a University. Participants 36 nursing students from two problem-based learning classes were recruited in two groups for the study. Methods After data collection, content analysis with axial coding approach was conducted to explore the narrative themes, to summarise the main ideas, and to make valid inferences from the connections among critical thinking, creativity, and other exogenous variables. Results Based on the findings, six major themes were identified: “revisiting the meanings of critical thinking”; “critical thinking and knowledge: partners or rivals?”; “is critical thinking criticising?”; “revising the meanings of creativity”; “creativity and experien partners or rivals?”; and “should creativity be practical?”. Conclusions This study showed that learners had diverse perspectives towards critical thinking and creativity, and their debate on these two domains provided implications on nursing education, since the voices of learners are crucial in teaching. By closing the gap between learners and educators, this study offered some insights on nursing education in the new curriculum, in particular to co-construct nursing knowledge which is student-driven, and to consider students' voices towards understanding and applying creativity and critical thinking in nursing.

An international educational experience can enhance the curriculum of an undergraduate program in nursing and can promote student development. However, many students who participate in international programs experience the debilitating effects of symptoms that comprise a phenomenon that is sometimes labeled culture shock. The progress of American nursing students through the stages of adaptation to a different culture is examined through journal entries the students recorded while they were in England for a short-term experience in community health nursing. The benefits of participation in an international program are discussed. Also included are guidelines for faculty to help them recognize the symptoms described and to implement appropriate interventions.

Pitfalls, perils and payments: Service user, carers and teaching staff perceptions of the barriers to involvement in nursing education

Background There is an impetus to involve service users and carers in the education of nurses and a general consensus in the literature about the benefits that this brings to all involved. Whilst these benefits are well rehearsed in the literature there is little written about the potential barriers to service user and carer involvement in nurse education. Objective The objective of this study was to investigate service users, carers and staff views on the potential barriers to becoming engaged in nurse education. Design A qualitative study using focus group discussions (FGD) was used to canvas the views of service users, carers and teaching staff. Setting A large school of nursing in the North West of England. Participants 38 service users and carers recruited from the North West of England and 23 nursing and midwifery teachers and lecturers. Methods Focus group discussions were employed as the main data collection method. The data were analysed using thematic analysis. Results Six themes occurred in the data as being negatively associated with potential and actual involvement: not knowing the context of the group, lack of preparation of the group, not being supported, not being allowed to be real, not receiving feedback, not being paid appropriately. Conclusions The process of involvement is not without difficulties. These data show that some consideration needs to be given to the potential barriers to involvement if the engagement of service users and carers is to be effective.

Critical thinking and creativity in nursing: Learners' perspectives

Background Although the development of critical thinking and the development of creativity are major areas in nursing programme, little has been explored about learners' perspectives towards these two concepts, especially in Chinese contexts. Objectives This study aimed to reveal nursing learners' perspectives on creativity and critical thinking. Design Qualitative data collection methods were adopted, namely group interviews and concept map drawings. Settings The process of data collection was conducted in private rooms at a University. Participants 36 nursing students from two problem-based learning classes were recruited in two groups for the study. Methods After data collection, content analysis with axial coding approach was conducted to explore the narrative themes, to summarise the main ideas, and to make valid inferences from the connections among critical thinking, creativity, and other exogenous variables. Results Based on the findings, six major themes were identified: “revisiting the meanings of critical thinking”; “critical thinking and knowledge: partners or rivals?”; “is critical thinking criticising?”; “revising the meanings of creativity”; “creativity and experien partners or rivals?”; and “should creativity be practical?”. Conclusions This study showed that learners had diverse perspectives towards critical thinking and creativity, and their debate on these two domains provided implications on nursing education, since the voices of learners are crucial in teaching. By closing the gap between learners and educators, this study offered some insights on nursing education in the new curriculum, in particular to co-construct nursing knowledge which is student-driven, and to consider students' voices towards understanding and applying creativity and critical thinking in nursing.

‘Closing the gap’ – A partnership approach to community care education for long term conditions

Aim The aim of the project/evaluation was to establish a Community of Practice (CoP) with the purpose of enhancing the skills of the workforce and developing learning resources to improve service delivery for people with long-term conditions. Background International and national policy drivers espousing new community-based inter-agency models of delivery have led to rapid transformation of services for people with long-term conditions. Many in the workforce lack the skills required to respond to the increased acuity of patients or function within a fully integrated, inter-agency service. Design A partnership involving a community provider, a commissioner and a University initiated a ‘CoP’ project with the aim of promoting knowledge-exchange between academic and practice staff. The CoP was the vehicle for addressing gaps in knowledge and skills of the academic and practice workforce. An evaluation of the CoP was conducted after 1year. Method The evaluation sought to explore how the CoP shaped the project's impact, the resultant changes in clinical practice and how involvement influenced participants' knowledge of new models/approaches. The CoP was made up of 34 academic and practice staff and was a forum for building collective knowledge that would shape practice. Qualitative data collection involved a focus group, questionnaire survey and notes of the CoP meetings. Findings Participants became more conversant with integrated models of care and identified and addressed gaps in knowledge and skills including real world complexity to reduce escalation of care. Participants found a well-facilitated CoP to be a supportive and effective vehicle for helping a workforce change process. Conclusion A well-organised Community of Practice, delivered as an inter-agency partnership, was found to be very effective in contributing to a better-prepared workforce. With targeted investment a facilitated CoP appears to be a useful vehicle for addressing workforce needs in complex and challenging circumstances.

Increasing numbers of people across the world are developing preventable non-communicable diseases (NCDs). This has massive social and economic implications for individuals, families, communities and countries, and impacts on the roles, responsibilities and future careers of nurses. The growing epidemic of NCDs is attracting increasing attention at every level and in almost every country. Many of the causes and solutions are beyond the remit of the health sector alone, calling instead for a system-wide approach. However, with the necessary knowledge and skills, the millions of nurses worldwide are well placed to make a significant contribution to the prevention and management of NCDs, focusing particularly on tobacco cessation, a healthy diet (including safe use of alcohol) and physical activity.

Exploration of the attitudes of nursing students to peer physical examination and physical examination of patients

Background The use of peer physical examination (PPE) in early clinical skills has been studied amongst medical students. The majority of students are comfortable with using peer physical examination, when sensitive areas are excluded. Students' attitudes are related to their personal characteristics: gender, age, religious faith, and ethnicity. There is no data on nursing students' attitudes to peer physical examination. Objectives Identify and explore: attitudes of nursing students towards PPE levels of comfort towards examination of patients relationships between demographics and attitudes to physical examination. Design Dual cohort, cross-sectional, anonymous survey. Setting Three-year undergraduate nursing programme, skills centre and service clinical learning. Methods All first and third year nursing students were asked to complete a modified Examining Fellow Students questionnaire at the end of 2008. The questionnaire asked students to indicate which of 12 body areas they would not be willing to examine/have examined by a peer of the same/opposite gender. This study also asked students which of the 12 body areas they felt uncomfortable examining on patients. Results The response rate was 76% (128/168). The students were predominantly female (93% female; 7% male). Most students were comfortable with examining non-sensitive body regions of peers (78.2%–100% willing) and patients (92.3–100% willing). Male gender was significantly associated with willingness to examine and be examined by peers (p=0.001); Asian students were significantly less willing to engage in peer physical examination with opposite gender (p<0.007). Year 3 students were significantly more comfortable than Year 1 in examining patients of either gender (p<0.001). Discussion and Conclusions In spite of the male gender findings, this predominantly female population expresses similar attitudes to the gender-balanced medical student studies — high acceptability for non-sensitive areas. The role of characteristics and attitudes to peer physical examination shows similarities and differences to other studies. Student characteristics were not related to patient examination attitudes.
