Developing a pedagogy for nursing teaching–learning

Each nurse educator's pedagogy underpins their understanding of and approach to teaching and learning, regardless of whether this has been reflected upon or articulated. In this paper, we overview factors and issues that should be considered when developing a teaching philosophy of nursing education and set out broad differences between traditional and contemporary pedagogic models and various ways of knowing. As values underpin any teaching framework these are considered in relation to pedagogies, epistemologies and their relevance to nursing practice. Key teacher roles and strategies that are congruent with a contemporary pedagogy for teaching nursing in the classroom or the clinical setting are also outlined. A premise for writing this paper was that clarifying one's own understandings of education and knowledge and the implicit values held within those terms and processes will contribute to greater self-awareness and more effective teaching of nursing. Education approaches underpinned by a sound teaching philosophy and framework can facilitate an educationally sound and positive experience for learners.

The evolution of midwifery education at the master's level: A study of Swedish midwifery education programmes after the implementation of the Bologna process

In Europe, midwifery education has undergone a number of reforms in the past few decades. In several countries, it has shifted from vocational training to academic education. The higher education reform, known as the “Bologna process” aimed to create convergence in higher education among a number of European countries and enhance opportunities for mobility, employment and collaborative research. It also indicated a transparent and easily compared system of academic degrees, generating a new educational system in three cycles. This study explores the implementation of the process in Sweden when the midwifery education was transferred from diploma to postgraduate or master's level. The aim of this study was to analyse how the implementation of the Bologna process in the Swedish higher education system has impacted midwifery education programmes in the country. Descriptive statistics and content analysis were employed to analyse 32 questionnaire responses from teachers and the 2009–2010 curricula and syllabi of 11 postgraduate midwifery education programmes at Swedish universities and university colleges. The results revealed variations among the universities at the major subject into the three disciplines; midwifery, nursing and caring with different conceptualisations, even when the content was identical in the curricula to that of the midwifery professional knowledge base. Implementation of the new reform not only has accelerated the academisation process, but also puts higher demand on the students and requires higher competencies among teachers to involve more evidence-based knowledge, seminars, independent studies and a postgraduate degree project in the major subject. Thus the students earn not only a diploma in midwifery, but also a master's degree in the major subject, which affords the opportunity for an academic career. But still there is a tension between professional and academic education.

Abstract
Objective: To develop and test a decision-support tool for prioritizing new competing Health Technologies (HTs) after their assessment using the mini-HTA approach. Methods: A two layer value/risk tool was developed based on the mini-HTA. The first layer included 12 mini-HTA variables classified in two dimensions, namely value (safety, clinical benefit, patient impact, cost-effectiveness, quality of the evidence, innovativeness) and risk (staff, space and process of care impacts, incremental costs, net cost, investment effort). Weights given to these variables were obtained from a survey among decision-makers (at National/Regional level and hospital settings). A second layer included results from mini-HTA (scored as higher, equal or lower), which compares the performance of the new HT (in terms of the abovementioned 12 variables) with the available comparator. An algorithm combining the first (weights) and second (scores) layers was developed to obtain an overall score for each HT, which was then plotted in a value/risk matrix. The tool was tested using results from the mini-HTAs for three new HTs (Surgical Robot, Platelet Rich Plasma, Deep Brain Stimulation). Results: No significant differences among decision-makers were observed as regards the weights given to the 12 variables, therefore, the median aggregate weights from decision-makers were introduced in the first layer. The dot plot resulting from the mini-HTA presented good power to visually discriminate between the assessed HTs. Conclusion: The decision-support tool developed here makes possible a robust and straightforward comparison of different competing HTs. This facilitates hospital decision-makers deliberations on the prioritization of competing investments under fixed budgets.

A framework for effective collaboration: A case study of collaboration in nursing education in the Western Cape, South Africa

A fundamental purpose of mergers between higher education institutions (HEIs) in 2002 was to enable sharing of scarce resources between more advanced universities and those historically disadvantaged by the apartheid system of the South African Government. A common teaching platform for undergraduate nursing education in the Western Cape was established in 2005, in line with the transformation of the higher education system, as a collaborative initiative between three universities. In order to evaluate the common teaching platform, Stuffelbeam's context, input, process, product (CIPP) research model was employed. A sample of 108 participants was selected through stratified purposive sampling, and included three deputy vice-chancellors, three deans, three heads of department, 18 lecturers and 81 students. Semi-structured interviews were held with the staff members, whilst the students participated in focus group interviews. Open-ended questions informed by literature and the CIPP evaluation model were developed and used to guide the interviews. This enabled the researcher to obtain a rich description of the participants' experiences. The data were analysed inductively. The results revealed that the main purpose of collaboration was not achieved due to the lack of a common understanding of the concept of collaboration and its purpose; a lack of readiness to collaborate and a lack of sharing of resources. A framework for effective collaboration was developed based on the results.

Personal change amongst mental health nursing students: A longitudinal study

Background Studies into the socialisation of nursing students have tended to concentrate on entrant's reactions to work and on the development of knowledge and skills in relation to a particular occupational culture. However, the extent and manner of personal and psychological changes experienced by mental health nursing students during occupational socialisation have yet to be thoroughly addressed in the literature. Objectives To explore the psychological and personal changes of student mental health nurses over the first 2years of their pre-registration mental health nursing programme. Design Students from 2 mental health nursing cohorts at a London university were invited to participate. A semi-structured interview was used and the participants were asked to talk openly about their experiences. Ethics approval and informed consent was sought and obtained. Participants 20 students were selected at random and invited to be interviewed on 5 occasions over a 2year period at roughly 6month intervals. Methods A total of 72 interviews were conducted. Interviews were tape recorded and verbatim transcribed for quantitative and qualitative content analyses. Results Participants described moving from feeling uncertain, to increasing awareness and understanding of self and others, and, ultimately, feeling more accomplished in relation to their occupational world. Conclusions This study has highlighted the need for supporting students in developing their confidence and perceptions of self-efficacy and of recognising individual differences in student's responses to uncertainty.

A descriptive survey investigating pre-registration student nurses' perceptions of clinical skill development in clinical placements

Background Clinical skill development is essential to nurse education. Clinical skills are frequently taught in higher education institutions using clinical simulation. It is unclear if clinical skills are subsequently consolidated and developed in clinical placements. Objectives The aim of this survey was to evaluate pre-registration student nurses perceptions of the frequency of opportunities to practise, the level of supervision and assessment of, clinical skills in their clinical placements. Design This was a cross-sectional survey design using an online, self-report questionnaire including a Likert-type scale and open ended comments. Participants Four hundred and twenty one students, from all year groups, from a university in the south of England on a wide variety of clinical placements participated. Methods Participants evaluated the frequency of opportunity to practise, level of supervision and assessment of and feedback on performance of specific clinical skills. Clinical skills evaluated were measurement of vital signs, aseptic non-touch technique, assisting with eating and drinking, and assisting with comfort and hygiene. Data were analysed utilising Statistical Package for the Social Sciences Version 19. Results The frequency of opportunities to practise skills in clinical placement was variable with some participants reporting that they never had opportunity to practise essential skills. Similarly the level of supervision and assessment was also inconsistent suggesting that participants frequently practised clinical skills unsupervised without being assessed as competent. Conclusions Inconsistencies in clinical skill development may lead to graduates who are not work ready and as a result, insufficient clinical competence potentially leads to unsafe practice and poor patient care. This calls for stronger partnerships between educators and clinical areas and the prioritisation of mentor preparation and education as well as organisational support in terms of mentor workload planning.

Nursing students' knowledge and attitudes towards dementia — A questionnaire survey

Background In line with population ageing, the number of individuals with dementia is expected to increase. Nursing students are more likely to care for dementia patients during their clinical placements and once they qualify. Nevertheless, they may not be adequately prepared during their undergraduate programme. Furthermore, lack of support during their placements may negatively influence nursing students' attitudes. Measuring the level of knowledge and attitudes of nursing students could be an important step in providing evidence on the need of enhancing dementia care training in the nursing curriculum. Objectives The aim of this study was to assess knowledge and attitudes of nursing students in Malta towards dementia and identify factors related to these attributes. Moreover, the students were asked to rate their training and educational needs from a list of topics about this subject area. Design A questionnaire survey. Setting Department of Nursing within the Faculty of Health Sciences at the University of Malta. Participants 280 full-time diploma and degree nursing students who are currently in their first, second and third year of their undergraduate programme. Method The data was collected using a questionnaire consisting of the Alzheimer's disease Knowledge Scale and the Dementia Attitude Scale. A 3-point Likert scale was also used by students to rate their training and educational needs from a list of 20 topics related to dementia care. The data was analysed using quantitative methods. Results Maltese nursing students had an adequate knowledge and showed positive attitudes towards Alzheimer's disease and dementia patients. Age, academic year, training and previous care of dementia patients during their clinical placement were all found to be associated with increased knowledge and positive attitudes. Conclusions Knowledge and attitudes of nursing students in Malta towards persons with dementia could be improved by enhancing dementia care training and improving the clinical experience.

Abstract
There has been an increased focus on the relationship between health technology assessment (HTA) and regulatory assessments and how regulatory, HTA and coverage bodies, and industry can work better together to improve efficiency and alignment of processes. There is increasingly agreement across sectors that improved communication and coordination could contribute to facilitating timely patient access to effective, affordable treatments that offer value to the health system. Discussions on aspects of this relationship are being held in different forums and various forms of coordination and collaboration are being developed or piloted within several jurisdictions. It is therefore both timely and of value to stakeholders to describe and reflect on current initiatives intended to improve interactions between regulatory, HTA and coverage bodies, and industry. Drawing on 2011 meetings of the HTAi Policy Forum and the Center for Innovation in Regulatory Science (CIRS), this study aims to describe and compare initiatives, and point to success factors and challenges that are likely to inform future work and collaboration.

The evolution of midwifery education at the master's level: A study of Swedish midwifery education programmes after the implementation of the Bologna process

In Europe, midwifery education has undergone a number of reforms in the past few decades. In several countries, it has shifted from vocational training to academic education. The higher education reform, known as the “Bologna process” aimed to create convergence in higher education among a number of European countries and enhance opportunities for mobility, employment and collaborative research. It also indicated a transparent and easily compared system of academic degrees, generating a new educational system in three cycles. This study explores the implementation of the process in Sweden when the midwifery education was transferred from diploma to postgraduate or master's level. The aim of this study was to analyse how the implementation of the Bologna process in the Swedish higher education system has impacted midwifery education programmes in the country. Descriptive statistics and content analysis were employed to analyse 32 questionnaire responses from teachers and the 2009–2010 curricula and syllabi of 11 postgraduate midwifery education programmes at Swedish universities and university colleges. The results revealed variations among the universities at the major subject into the three disciplines; midwifery, nursing and caring with different conceptualisations, even when the content was identical in the curricula to that of the midwifery professional knowledge base. Implementation of the new reform not only has accelerated the academisation process, but also puts higher demand on the students and requires higher competencies among teachers to involve more evidence-based knowledge, seminars, independent studies and a postgraduate degree project in the major subject. Thus the students earn not only a diploma in midwifery, but also a master's degree in the major subject, which affords the opportunity for an academic career. But still there is a tension between professional and academic education.

Aim The aim of the research was to evaluate the effectiveness of implementing the Respiratory Skills Update (ReSKU) education program using integrated teaching and learning strategies, in the context of organisational utility, on improving surgical nurses' practice in the area of respiratory assessment. Background Technological advances and changes in healthcare delivery have necessitated that nurse educators adopt innovative teaching and learning strategies to better prepare acute care nurses for their increasingly complex roles. This 2007 study used a robust overarching theoretical framework to develop and evaluate an educational model using the ReSKU program as a basis for the content. Methods The study was conducted in a 400 bed regional referral public hospital, in Australia. The research was guided by the work of Forneris (2004) to operationalise a critical thinking process incorporating the complexities of the clinical context. The theoretical framework used multi-modal, interactive educational strategies that were learner-centred and participatory. These strategies aimed to engage the clinician in dynamic thinking processes in clinical practice situations guided by coaches and educators. Conclusion The construct of critical thinking in practice combined with clinical reasoning and purposeful and collective reflection is a powerful educational strategy to enhance competency and capability in clinicians.
